SN16: Systemische Entwicklungsdiagnostik und Therapie bei Säuglingen und Kleinkindern (Systemic Development Assessment and Therapy with Infants and Toddlers) / Deutsch

Systemische Beratung bei chronisch kranken Kindern (Systemic Consultation for Chronic Impairments in Infancy)

Anhand einer qualitativen Studie werden Tendenzen der Entwicklung bezüglich systemischer Beratung bei chronisch körperlichen Erkrankungen im Kindes- und Jugendalter aufgezeigt. Dabei geht es insbesondere um den Zugang zu den jugendlichen Patienten und ihren Familien, aber auch um Möglichkeiten der Behandlung unter systemischen Gesichtspunkten neben medizinischen Therapien. Dafür werden spezifische systemische Techniken heraus gestellt, die sich in medizinischen Kontexten als praktikabel erwiesen haben. Gegenstand der Untersuchung sind zehn qualitative Experteninterviews im deutschsprachigen Raum, die nach dem Ansatz der Grounded Theory (Glasser/Strauss) ausgewertet wurden. Es lassen sich Tendenzen einer Institutionalisierung und einer Professionalisierung des systemischen Ansatzes in medizinischen Kontexten feststellen. Ferner werden Perspektiven aufgezeigt, wie systemisches Denken und Handeln unter bestimmten Bedingungen Einzug in medizinische Kontexte halten kann bzw. bereits gehalten hat. Auf Basis der qualitativen Studie wird ein systemisches Modell für chronisch körperlich erkrankte Kinder und Jugendliche entwickelt. Es bleibt abzuwarten, inwiefern sich der systemische Ansatz weiterhin auf medizinische Bereiche auszudehnen vermag und hinsichtlich der Effektivität und Effizienz des Gesundheitswesens einen Beitrag zu leisten hat.

Emotionale Verfügbarkeit in einer Stichprobe der psychotherapeutischen „Baby- und Kleinkindsprechstunde“ der Charité (Emotional Availability in a Sample of the Psychotherapeutic Baby- and Infant Consultation of the Charité)

Die diagnostische Klassifikation mit Hilfe der ICD-10 und der DC 0-3 (Diagnostische Klassifikation von 0 bis 3 Jahren) wurde untersucht. Zur Einschätzung der Eltern-Kind Interaktion wurden die Skalen zur Emotionalen Verfügbarkeit eingesetzt. Bei 68 Eltern-Kind Paaren wurden die Daten der standardisierten Videointeraktionsbeobachtung auf Korrelation mit der diagnostischen Klassifikation und anderen soziodemographischen Daten überprüft. Die Eltern Kind Interaktion gemessen mit den „Emotional availability scales“ zeigt signifikante Unterschiede in Abhängigkeit von der Diagnose. Weniger gut ausgeprägte emotionale Verfügbarkeit geht linear mit intensiverem therapeutischem Bedarf einher. Die Einschätzung der Eltern-Kind-Interaktion ist ein zentraler Bestandteil der Diagnostik und Therapie psychischer Störungen in der frühen Kindheit.

SN17: Stationäre systemische Kinderpsychiatrie (Systemic Child and Adolescent Psychiatry in an In-Patient Setting) - Deutsch

Wie baut man eine systemische Kinder- und Jugendpsychiatrie? (How to Build up a Systemic Child and Adolescent Psychiatric Department?)

In dem man es tut. So könnte man es erst einmal stehen lassen. Doch ganz so einfach ist es nicht. Der Anfang ist gar nicht so schwer, da es zunächst „nur“ darauf an kommt die richtigen Mitarbeiter ein zu stellen. Aber den systemischen Aspekt dauerhaft nicht aus dem Auge zu verlieren, ist nicht mehr so einfach. Manchmal droht er im Alltagsgeschäft unter zu gehen. Vor allem wenn das Systemische noch die Zusätze „lösungs- und ressourcen orientiert“ sowie „konstruktivistisch“ bekommt. Der Vortrag sollte Ein- und Ausblicke in den Alltag einer solchen Abteilung am Marienkrankenhaus Papenburg-Aschendorf geben und Lust machen es auch aus zu probieren.

Kooperation mit anderen Helfern (Co-operation with Other Professional „Helpers”)

Kooperation ist sowohl innerhalb als auch ausserhalb unserer Klinik ein wesentlicher Bestandteil in unserem alltäglichen Handeln, das bedeutet, Kooperation mit anderen Helfern ist eine Grundvoraussetzung für unser systemisches, lösungsorientiertes Arbeiten.
Effektive Zusammenarbeit setzt verschiedene Fähigkeiten und Ressourcen von den Beteiligten voraus, welche es zu verknüpfen gilt.
In diesem Kurzreferat werden wir unter Berücksichtigung der Vorgenannten Fähigkeiten und Ressourcen verschiedene Formen der Zusammenarbeit vorstellen. Zum einen die Zusammenarbeit innerhalb unseres Klinikteams und zum anderen unsere Zusammenarbeit mit externen Systembeteiligten.

Ressourcenorientierte Teamentwicklung (Teamdevelopment from a Solution-Focused Point of View)

In diesem Vortrag wird über die praktischen Erfahrungen, Möglichkeiten und Herausforderungen referiert, die wir beim Aufbau einer systemisch konzipierten Kinder- und Jugendpsychiatrie im Laufe von 10 Jahren sammelten. Es wird thematisiert, wie multiprofessionelle lösungsorientierte Angebote ein systemisches Milieu kreieren können. Stationäre Therapie wird in diesem Sinne konzipiert, als zur Verfügung stellen eines wachstumsfördernden Übungsfeldes für Kinder, Jugendliche und deren Familien. Metaphorisch kann das „Behandlungsziel“ als das „Weben eines Ressourcenteppichs“ beschrieben werden.
Des weiteren berichten wir über Erfahrungen, Ideen und Strategien einer ressourcenorientierten Teamentwicklung nach dem Motto „Pflege des Teamgeistes vs. Gespenst des Burn-out“.

Visionen aus der Sicht der Helfer und der Kunden (Visions from the View of Professionals and Customers)

Anhand einer Videopräsentation werden wir die Ergebnisse einer Befragung hinsichtlich der Visionen einer systemischen Klinik für die nächsten 10 Jahre aus der Sicht der Mitarbeiter zum Einen, zum anderen aus der Sicht unserer Kunden, das heisst, Patienten und ihre Eltern präsentieren.
Im Anschluss daran sollen diese Visionen einmal durch die vortragenden Experten und zum Anderen durch ein Expertenteam, was aus dem Zuhörerkontext vor Ort zu bilden ist, gemeinsam in Form eines reflektierende Teams reflektiert und diskutiert werden.

The Construction of Identity in Agoraphobia: the Influence of Family (Die Identitätskonstruktion in der Agoraphobie: Der Einfluss der Familie)

The study will analyze, through qualitative methodology, the narrative identities of people diagnosed with agoraphobia and will be compared with people without diagnostic. The aim is to establish in the first place, if is there a prototypical narrative of agoraphobia. The second objective is to corroborate the influence of family relations in the construction of identities. The instruction for the narrative research will be: “Describe yourself”, “How did you become the way you are” and “What was the influence of your family in that process”.

Story-Telling Aids in Individual Systemic Psychotherapy: A Case Example (Hilfestellungen zum Geschichtenerzählen in der systemischen Einzeltherapie: Eine Falldarstellung)

A number of ‘old’ tools, adapted to facilitate story-telling, may be used in systemic psychotherapy with individuals especially those with serious psychological problems. The author presents the use of drawings, fiction and early recollections in the continuing long-term therapy of a young woman with borderline symptoms. The author has been interested in the adaptation of ‘old’ tools from other psychotherapy approaches, providing a family and narrative rationale for practice. In this presentation, such adapted tools are seen as useful aids in giving voice to various aspects of stories that cannot be easily verbalized because they feel too threatening for both the self and the family of the client. One of the biggest advantages in using such means to facilitate stories is that new therapy perspectives can be based on material coming from the client herself. These perspectives then feel safer to explore. Also, the therapist and the client may come back to the material at different times. This helps achieve a more comfortable therapy pace and to monitor changes collaboratively.

Narratives of Depression in Couple Therapy (Narrative der Depression in der Paartherapie)

The narratives of depressed patients told in couple therapy were analysed to find the couples' ways of co-constructing their separate and shared stories of depression. Three couples, in which one of the spouses had been referred to an outpatient clinic of a university hospital for treatment for his/her depression were chosen for the study. Couple therapy was the treatment for their depression. Four sessions from each couple therapy processes were videotaped, transcribed and analysed by grounded theory - based narrative analysis. The depressed spouse’s narratives had two dimensions: individual and interactional. The non-depressed spouse’s narratives had three dimensions: they either supported, contradicted or enriched the depressed spouse’s narrative. The results show that in the couple therapy of depression it is crucial to focus not only on (1) the patient’s individual narrative of depression, but also on (2) depressed spouse’s narrative as an interactive part of non-depressed spouse’s narrative and on (3) the shared narrative created by the spouses together.

Selbstorganisation in der Therapie: Erinnerungen, Wahrnehmungen und Zukunftskonstruktionen (Self Organisation in Psychotherapie: Remembrances, Perceptions and Constructions of Future)

Fragestellung: Selbstorganisation als Konzept spielt eine wichtige Rolle in den Theorien der Dynamischen Systeme, der Synergetik, der Fraktale, und der Autopoiesis. Angemessene Berücksichtigung von Komplexität sowie nahezu universelle Anwendbarkeit in zahlreichen sehr unterschiedlichen Fachgebieten wie z. B. Mathematik, Industrie, Soziologie, Neurobiologie und Psychotherapie gehören zu seinen Stärken.
Design und Methode: Aspekte der Selbstorganisation bei der Entstehung von Erinnerungen, Wahrnehmungen und Zukunftskonstruktionen werden in einem psychotherapeutischen Kontext diskutiert unter Berücksichtigung entsprechender neurobiologischer Konzepte.
Resultate: Die Berücksichtigung von Konzepten der Selbstorganisation unterstützt fachgebietübergreifendes integratives Denken und Handeln, erleichtert die Reflektion therapeutischer Strategien und ermöglicht die Integration von älteren und neueren systemischen Methoden und Ansätzen in ein zeitgemäßes Behandlungskonzept.

Schaukel-Prozesse in die Psychiatrie: Über die Notwendigkeit anderer System-Modelle im Umgang mit vielfältiger psychiatrischer Problematik (Swing Processes in Psychiatry: On the Necessity of different System Models in the Treatment of Multiple Psychiatric Problems)

Fragestellung: Psychiatrische Störungen sind vielfältig und komplex. Viele Faktoren (neurobiologische, psychologische, soziale, Drogen, Traumata, Beziehungsnetzwerk) beeinflussen ihre Verläufe. Oft sind auch mehrere Helfer und Institutionen gleichzeitig beteiligt. Die klassische System-Modelle sind ungeeignet, die komplexe Vielfältigkeit von Interaktionen und Perspektiven zu beschreiben und therapeutisch zu nutzen. Ihr Fokus auf Geschlossenheit, Homöostase und Selbstorganisation reicht nicht, die rekursive Dynamik in und zwischen mehrere Kontexten zu charakterisieren. Auch die Systemmodelle, die auf biologische oder naturwissenschaftliche Metaphern zurückgreifen, sind inadäquat oder zu abstrakt. Außerdem vernachlässigen diese Modelle den Anteil der Zuschauer selbst in den Beschreibungen von Interaktionsprozessen. Design and Methode: Die Komplexitätstheorie und Dynamische System-Theorie bieten bessere Anknüpfungspunkte für die Beschreibung der vielfältigen Dynamik in psychiatrische Kontexten. In meinem Vortrag benutze ich Konzepte aus diesen Theorien für den therapeutische Umgang mit sogenannten Schaukel-Prozessen in Interaktionen zwischen Patienten, Therapeuten und Instituten.

Störungsspezifische Ideen im Kontext einer systemischen Klinik (Disorder-Specific Ideas within the Context of a Systemic Clinical Department)

In diesem Kurzreferat werden wir anhand der Anorexia Nervosa und der posttraumatische Belastungsstörung in praktischen Beispielen zeigen, wie lösungs- und ressourcenorientiert mit diesen beiden Störungsbildern umgegangen wird. Die von Günther Schiepek beschriebenen systemisch Grundpositionen ( Berücksichtigung der Autonomie von Systemen, Veränderungen innerer Konstrukte und Wirklichkeitskonstruktionen sowie wechselseitiger Bezug zwischen individuellen Problemen und interpersoneller Kommunikation ) sollen im Hinblick auf die beiden Störungsbilder diskutiert werden.

Einblicke in das selbstorganisierende Gehirn mit bildgebenden Verfahren (Does Functional Brain Imaging (fMRI) Give Some Relevant Informations on the Self-Organizing Brain?)

Der Vortrag thematisiert zwei Schwerpunkte neurowissenschaftlicher Untersuchungen der Psychotherapie. Erstens: Gibt es Chancen, nicht nur den Veränderungen im individuellen Gehirn, sondern auch in sozialen Interaktionsmustern mittels neurobiologischer Methoden näher zu kommen? Einige Beispiele sollen deutlich machen, dass es diese Chancen gibt. Zweitens: gibt es Chancen, mit der Methodik bildgebender Verfahren (insbesondere fMRI) selbstorganisierende Prozesse im Gehirn zu studieren (d.h. Ordnungsübergänge und kritische Fluktuationen von Kognitions-Emotions-Verhaltens-Mustern)? Es wird dargestellt, wie derartige Untersuchungen aussehen müßten und wie die Methodik des fMRI in Prozessuntersuchungen von Psychotherapien integriert sein müßten.

SN20: Family Research and Psychiatry in Different Contexts. A Presentation of the Family Research and Intervention Section of the World Psychiatric Association WPA (Familienforschung und Psychiatrie in unterschiedlichen Kontexten. Ein Seminar der Sektion für Familienforschung und -intervention bei der World Psychiatric Association WPA)

Family Therapy and Family Functioning in Patients with Mood Disorders (Familientherapie und Familiendynamik bei Patienten mit affektiven Störungen)

We examined the impact of adjunctive family therapy on the functioning of families of patients with major depression and with bipolar disorders. Method: Data are presented from two treatment studies 1) 92 patients with bipolar disorder were randomly assigned to three treatment conditions: pharmacotherapy alone (PT), PT + family therapy; PT + multifamily psychoeducational group therapy and 2) 121 depressed inpatients were randomly assigned to follow-up care in four treatment conditions: medication + clinical management (MCM); cognitive therapy (CT) + MCM; family therapy (FT) + MCM; and CT + FT + MCM. Family therapy and subjective and objective measures of family functioning were based on the McMaster Model of Family Functioning. Results: Bipolar patients with poor family functioning at index episode significantly improved their family functioning in all but one dimension by month 28. Even patients with good family functioning at index episode significantly improved their family functioning in three dimensions. Improvement in family functioning was not related to symptom reduction ( 2(l)=. 191,ns) whether measured by a priori (Bech-Rafaelson and Hamilton Depression Rating) or post hoc (median split) definitions. Improvement was related to receiving family treatment. Depressed patients with poor family functioning significantly improved their family functioning by 6 months and were able to sustain the improvement through 18 months. Patients with good family functioning also improved by 6 months but then lost some of the gains. Improvement in family functioning was not related to improvement in symptoms (t (86. 0)=-1. 10,ns) based on a 50% reduction in Hamilton Depression Rating Scores. Improvement in family functioning (by number of family dimensions that improved significantly and by level of significance) was related to receiving family therapy. Conclusions: Despite improvement in mood symptoms, pharmacotherapy alone does not lead to improvement in family functioning in patients with mood disorders. Adjunctive psychosocial (especially family) interventions were related to significant improvement in family functioning, particularly in families experiencing the greatest distress.

Alcoholic Families: Functional Typology Revisited. A Multimodal Research Including Family Functioning - McMaster Model - on Hungarian Alcoholic and Non-Alcoholic Sample ("Alkoholfamilien": eine funktionale Typologie auf dem Prüfstand. Eine Multimodale Studie zur Familiendynamik (McMaster-Modell) bei ungarischen Samples aus Alkoholikern und einer Kontrollgruppe)

Objective: The presentation conveys summary of a research of an urban population including 64 alcoholic members and 36 control healthy index persons and their families. One of the hypothesis was that the family members of alcoholics perceive their family functioning dysfunctional comparing to the healthy controls.
Methods: semi-structured multigenerational interviews, München Alcoholism Screening Test (MAST), socio-economic status questionnaire and FAD were used to measure the sample.
The McMaster Family Assessment Device (FAD) is a 60-item questionnaire that is based on the McMaster Model of Family Functioning. The model describes structural and organizational properties of the family group and the patterns of transactions among family members that have been found to distinguish between healthy and unhealthy families. The Hungarian version of FAD was implemented by Keitner at all. during l980-ies for major depression.
Results: in the sample the FAD differentiates between families with alcoholic members and healthy families. There were statistically significant differences in the dimension of Problem Solving, Communication, Affective Responsiveness and Affective Involvement between the alcoholic sample to the control, indicating more dysfunctional patterns in the alcoholic group. General Functioning score also differentiate significantly between the two groups.
Conclusion: the results support Steinglas’s hypothesis that the stable alcoholic family system adjust to the alcoholism and provides a more stable and accommodating environment for its alcoholic members. Alcohol abuse serves as an central organizing principle for the family's interactional life. Also consistent with Liepmann et alii’s conclusion that alcoholics and their mates perceived family functioning to be better during dry than wet periods. The FAD has been used to distinguish clinical (alcoholic) and nonclinical populations have been shown to differentiate healthy from unhealthy families, lower scores indicating healthier responses. The multimodal analysis indicates 3 types of clusters (3 types of alcoholic families) in this sample.

Systems Therapy Methods in Acute Psychiatry - SYMPA (SYMPA - Systemische Therapiemethoden in der Akutpsychiatrie)

SYMPA is a large action research project in Germany. It involves six psychiatric acute treatment wards in three hospitals, it runs from 2002 until 2006, and it is funded by the „Foundation for Education and the Promotion of the Handicapped“, Stuttgart and by the two German Societies for Systemic Therapy (DGSF and SG). It´s goal is to establish systemic thinking and practices as a common denominator for all psychiatric activities – from intake through diagnosis, medication and psychotherapy until treatment termination. 90 workers from these three hospitals and from all professions involved (nursing, occupational therapy social work, psychiatry, psychology) participate in a specifically designed, manualized,18 days training course led by the Heidelberg-based Helm Stierlin Institute. The presentation will give a brief overview of the clinical procedures taught and of how we cooperate with the hospitals and their workers

Exploring Meanings and Understandings in Families: Qualitative Research Methods in Family Therapy (Erforschung von Bedeutungsgebung und Verstehen in Familien: Qualitative Forschungsmethoden in der Familientherapie)

The presentation will offer an overview of some contemporary qualitative research methods and explore the potential of their application in research into family therapy. It will link current qualitative research methods with the pioneering studies of early systemic family therapy. This will include a consideration of how methods such as interpretative theme analyses, discourse analysis and narrative analysis can be employed within a systemic framework which includes a broader analysis of the dynamics of family communicational processes. Some connections to the observational and discourse methods currently employed in attachment theory research will also me made. Examples of research exploring the process of family therapy as well as a conceptual analysis of family dynamics will be outlined.

The Multiple Voices of Sorrow. The Oncologic Disease as a Narration (Die vielfachen Stimmen des Leidens. Die Krebserkrankung als Narrativ)

Objectives: Starting from the description of the research about the patients’ and his relatives’ styles of coping when cancer diagnosis is made, developed by the Institute of Medicine and Systemic Psychology in Naples, some guidelines for the building of the help relation as for chronic somatic patologies according to an integrated approach are stated (Health Counselling as the Systemic Relational approach).
The main objective of such a practise is to make sure that the monologue of sorrow, closed in the narrow bounds of body, can turn into a dialogue, so becoming a narration. A narration which can become a re-narration through the focalization and modification of the meanings connected to the disease.
Reference: The theoretic principles and applicatory implications of Bio-Psycho-Social Model, re-analized within a Systemic frame and the Theory of Complexity, are illustrated in this work.
Conclusions: The research puts in evidence the importance to create a help relation where the attention to the counsellor’s emotional implications is added to the work of the individuation and re-definition of the meanings attributed to the disease in the same way they appear through the narration. Such meanings refer to familiar mythology passed through generations, as well as to socio-cultural background, and they strongly influence both the perception of the disease as an event, and the patient’s and his relatives’coping styles.

The Systemic Hypothesis: a Factor Indicating and Promoting Change in the Therapeutic System (Die systemische Hypothese: ein Faktor zur Anzeige und Förderung von Veränderung im therapeutischen System)

Addition of new information through narratives and often through re-narratives of same events results in the formation of successive systemic hypotheses which act as diagnostic tools and also as indicators of the therapeutic process. In this view the systemic hypothesis functions as a continuously changing diagnostic factor and reflects the changes appearing in therapy. Successive hypotheses, based on the narrative material appearing in the course of therapy – both short and long term – complement each other, promote the continuum of the therapy, provide differentiation and give unity to change. The use of successive hypotheses, in the therapeutic journey of a group therapy member over a period of two years will be presented through specific examples.

Getting onto Family Rituals Using Narrative Techniques (Erfassung von Familienritualen mit narrativen Techniken)

Narrative therapies assume that the objective reality doesn’t exist, but only the stories that we construct from reality (White & Epston, 1990). Therapy, through a conversation or written document, can then be seen as a co-construction of new narratives that promote the psychological well being of the client.
Our service is integrated in a cultural context in which some festive periods are full of narratives and messages that transmit positive emotions.
In these periods it’s usual to exchange postcards and letters. Our service put this reality into the therapeutic context, using this kind of cultural rituals (following the ideas of Imber-Black, Roberts & Whiting, 1988) as narrative techniques. This could turn the therapy into a more continuous and effective process.
This intervention was used in Christmas, reconciling usual actions with the therapeutic intervention. For this purpose we sent Christmas postcards together with letters and books. The letters’ content was mostly therapeutic, referring subjects already discussed in therapy. As a way of cognitive and emotional stimulation, reflexive questions were also included. The books sent included stories, thoughts, poetry and suggestions to make a better day-to-day life, which allowed reflection and change to an optimistic thought.
We think that this way of intervention has been effective and it can allow a greater proximity between the family therapy context and the clients’ meaningful rituals.
